Sliding Scale Discount Program

Monticello Medical Associates, Inc. provides services to all patients regardless of a person’s ability to pay. To assist patients who may have difficulty paying, Monticello Medical Associates, Inc. offers a sliding discount program for our uninsured and under-insured patients who meet federal eligibility guidelines.

The Monticello Medical Associates, Inc. sliding-fee-scale discounts are based on the Federal Poverty Guidelines. If your family qualifies for discounts, you will be assigned to the appropriate sliding fee scale level based on your income and the number living in your household.

Monticello Medical Associates, Inc. partners with the following pharmacies to offer the 340B program, a program to provide discount pricing on medication.
